Lucas Release Forms

All Subcontractors, Sub-subcontractors, Suppliers and Vendors are required to submit only Lucas approved conditional and unconditional waiver and release forms along with their invoices. Below are the approved Lucas Waiver & Release Forms to be used on all Projects.

Lucas Insurance Requirements

All Subcontractors must meet our Standard Insurance Requirements prior to joining our bid list. Please forward a certificate of insurance as proof/evidence of insurance to be certain your Company prequalifies to partner with our team. Note: Some projects may require additional bonding or insurance limits or requirements that will be disclosed at the time of bidding.


Stormwater Pollution Prevention (SWPP)

All Subcontractors must agree to comply with our Stormwater Pollution Prevention Compliance Guide. This Guide is applicable to all of our projects and we require every Subcontractor return an Acknowledgement indicating their agreement to comply prior to accessing our jobsites.


Safety Manual / Illness and Injury Prevention Plan (IIPP)

All Subcontractors must agree to comply with our Safety Manual / Illness and Injury Prevention Plan (IIPP). This Guide is applicable to all of our projects and we require every Subcontractor return an Acknowledgement indicating their agreement to comply prior to accessing our jobsites.
